[HTML] Dreamweaver and a comment field

Ok. Im adding a profile page to my website, and I want to make it so people can leave comments on it if they so choose. Is there any string of code I can enter that would do the following:

1) Allow people to leave a comment on my profile page after the click on the "Leave a comment" link.
2) Provide an optional name, webpage, and email address field.
3) The comment, after the user enters it, will be displayed into a predetermined field I have specified on the page itself.
4) Anything else I'm forgetting.

So! What am I looking at?

you won't be able to do that with only html, you'll need some server side scripts connecting to a datasource to store and retrieve the previous comments
